Canada Personal Health Identification Number (PHIN)

ICD-10 Code

The International Statistical Classification of Diseases and Related Health Problems (ICD) is a classification list comprised by the World Health Organization (WHO) for the purpose of coding diseases, signs and symptoms, abnormal findings, complaints, social circumstances, and external causes of injury or diseases. This policy identifies content in the 10th revision of ICD.

ICD-10 Code and US Person Name

Combines identification of a US Person Name with content from the ICD-10 Code which must be within 30 terms of one another.

ICD-10 Code and US Social Security Number

Combines identification of a US Social Security Number and content from the ICD-10 Code which must be within 30 terms of one another.

Medical Prescriptions

Identifies information relevant to an Rx prescription, containing information such as drug name, person name, and/or dosage information. Each of the following must be included for a match:

  • Person Name
  • Drug Name
  • Any Number
  • One of the following keywords:
mgmilligrammlmillilitermililitermillilitremililitremil
  • One of the following keywords:
rxRxRXrX

The Person Name must be within 50 terms of a Drug Name, which is within 5 terms of a number followed by one of the qualifier keywords. The Rx keywords can appear anywhere in the document.

NDC Number

The National Drug Code (NDC) is used in the United States to identify drugs for human use. The code is ten digits separated into three segments. The policy searches for the variations of the segments and these proximity keywords related to NDC content:

US Person Name and Drug Name

Identifies assets that contain both a US person name and a common drug name. At least one of each type is required and they must be detected within 30 terms of each other.

US Person Name and Health Condition

Identifies assets that contain both a US person name and a health condition. At least one of each type is required and they must be detected within 30 terms of each other.

US Social Security Number and Drug Name

Identifies assets that contain both a US social security number and a common drug name. At least one of each type is required and they must be detected within 30 terms of each other.

US Social Security Number and Health Condition Name

Identifies assets that contain both a US social security number and a health condition. At least one of each type is required and they must be detected within 30 terms of each other.
